To complete enrollment in our Final Mile Data Link solution, please be prepared to provide the following information: **Company Information**   - Company Legal Name - Company Headquarters Address, including: Street Address, City, State (or non-us equivalent), Zip Code (or non-us equivalent), Country **Contact Information for your company's Primary Traceability Administrator** - First & Last Name, Email Address, Phone Number for the person who will act as your Trace Exchange account administrator) **Billing Information**   - First & Last Name, Phone, Email Address for the Billing Contact (associated with the billing method selected) - Street Address, City, State (or non-us equivalent), Zip Code (or non-us equivalent), Country - Credit Card Details: Credit Card number, expiration date, card security code - Or, ACH Details: Routing Number, Account Number, Account Type **You'll also need to answer the following questions:** - Do you have more than one location? - Do you have a central kitchen where you prepare and ship foods to other stores or restaurants? - Do you have warehouses that ship products on the food traceability list? - Do you repack and ship product? *If you answer yes or not sure to any of the questions above, we recommend purchasing a FSMA 204 compliance solution for your organization as a Final Mile Data Link will not enable you to be fully compliant.* **Saas Agreement Acceptance** To complete enrollment, you will also need to read, scroll to the end, and accept the SaaS agreement. Make sure the person completing enrollment is authorized to do so on behalf of your company, as the SaaS agreement is a legally binding document.
